For Families & Clients

What services does Radiant Care provide?
Individualized Home Supports, Respite Care, Night Supervision, and 24-Hour Care. We work with individuals with disabilities and with seniors, through waiver funding or private pay.
What areas do you serve?
The Twin Cities metro, including Hennepin, Ramsey, Dakota, Anoka, Washington, Scott, and Carver counties.
How do I get started?
Call us at 651-587-5052, or have your case manager send a referral through our referral form. We'll follow up to talk through what support looks like for your situation.
Do you accept private pay, or only waiver funding?
Both. We work with clients on CADI, DD, BI, and EW waivers, and we also offer private pay senior care for families who aren't using a waiver.
What should I expect at the start of services?
An initial visit to go over the support plan, discuss scheduling, and introduce staff before regular services begin. We tailor the approach to what's already in place for you or your family member.
Can I request a specific staff member?
We do our best to match based on needs and availability, and we take fit seriously — consistent staffing is something we prioritize. Let us know your preferences and we'll work with them where possible.
What if I need to change or cancel a scheduled visit?
Call our office as soon as you know. We'll work with you to reschedule or adjust coverage.

How do I refer a client?
Use our online referral form, or call us directly at 651-587-5052. We'll follow up promptly to begin intake.
What's your typical response time on a referral?
We aim to follow up within one to two business days of receiving a referral.
What counties and waivers do you accept?
Hennepin, Ramsey, Dakota, Anoka, Washington, Scott, and Carver counties. We work with CADI, DD, BI, and EW waivers, as well as private pay clients.
What information do you need to process a referral?
Client contact information, county, waiver type (if applicable), and the service being requested. Any relevant support plan details are helpful but can follow after initial contact.
Can you provide services alongside a client's other providers?
Yes. We regularly coordinate with other providers already involved in a client's care.
How do you handle urgent placement requests?
Call us directly rather than using the online form for anything time-sensitive — we can assess capacity and respond faster over the phone.
